Warm up phenomenon and preconditioning in clinical practice.

چکیده
The warm up phenomenon, first described more that 50 years ago in patients with effort angina, refers to the improved performance exhibited by more than half of all patients with coronary artery disease following their first exercise test. There are several potential causes of the warm up phenomenon including: (1) an improvement in oxygen supply, which, in turn, may be caused by stenosis dilation, collateral recruitment or myocardial perfusion redistribution; (2) a reduction in oxygen consumption caused by an adaptive down regulation or stunning of regional myocardial contractile function in the ischaemic region; and (3) a form of myocardial adaptation to ischaemia akin to ischaemic preconditioning.

Effects of K(ATP) channel blockade by glibenclamide on the warm-up phenomenon.
AIMS The increased tolerance to myocardial ischaemia observed during the second of two sequential exercise tests, i.e. the warm-up phenomenon, has been proposed as a clinical model of ischaemic preconditioning. As ATP-sensitive K+ channels appear to be a mediator of ischaemic preconditioning in both experimental and clinical studies, the aim of this study was to investigate the role of K(ATP) c...
متن کاملConsequences of Brief Ischemia: Stunning, Preconditioning, and Their Clinical Implications
In experimental studies in the dog, total proximal coronary artery occlusions of up to 15 minutes result in reversible injury, meaning that the myocytes survive this insult. The 15 minutes of ischemia, however, induce numerous changes in the myocardium, including certain monuments to the brief episode of ischemia that may persist for days. One of these monuments is stunned myocardium, which rep...

متن کاملIschemic preconditioning in humans: models, mediators, and clinical relevance.
Ischemic preconditioning, a powerful form of endogenous protection against myocardial infarction, has been demonstrated in several animal species and, recently, in isolated human cardiomyocytes. For both logistic and ethical reasons, no clinical study can meet the strict conditions of experimental studies on preconditioning with infarct size as the end-point. Nevertheless, the demonstration of ...

INTRODUCTION Cold storage of organs for preservation and transplantation is reaching its limits especially with extended criteria for heart beating donors and donation after cardiac death. We will discuss recent findings and perspectives in normothermic kidney preservation. METHODS A literature review was performed from original articles and syntheses selected by the search engine PubMed. Key...
